New Swimmer Forms

New members of the swim team must provide the following forms.

In order to tryout, a student must present a current  HISD Athletic Physical.  To be current and valid for the upcoming year, the physical must be on the new 2017 HISD physical form and be taken no earlier than May, 2017.

In order to tryout, the student must either buy the HISD Insurance for $35 (in case of injury) or have signed the 2017-18 Insurance Waiver Form (not available until July).  The 2017 HISD Insurance Waiver Form indicates that you already have insurance and waive the offer to buy HISD insurance.  The waiver form must be notarized and you must provide proof of insurance when signing it.

In order to participate on an athletic team, any new student to Bellaire HS (including zoned rising 8th graders) must complete a Previous Athletic Participation Form.  This form must have 2 signatures form administrators or coach at the student's previous school.  Please try to get this form completed before August when the schools become busy with enrolling new students.  This form must be completed regardless of whether the student participated in sports at the previous school and regardless of whether they are zoned or not to Bellaire.

Finally, there is a packet of forms to be signed.  The first form, the Parent Approval Form must be notarized.  The other forms require signatures of students and parents only.

SWIM TEAM TRYOUTS FOR 2017-2018

Tryouts for the Bellaire HS Swim Team will be held on Thursday, August 17, 2017 at 4:30 pm at the Bellaire High School pool.
Make-up Tryouts will be held Monday, August 21, 2017 at the BHS at 4:30 pm.
Bring your own swim suit, goggles and towel.
You must also bring a copy of a current athletic physical.
